Application for Registration & Licensing without Exam STEPS IN FILING APPLICATION FOR REGISTRATION WITHOUT EXAMINATION
1.) Accomplish Application Form.
2.) Submit accomplished form together with required documents to Application Division at the PRC Central Office or at the Regional Office Processing Counters.
3.) Sign on the PERRC Issuance Control Form.
4.) After three months, verify status of application at Telephone Number (632) 314-0027 or email at appdiv@prc.gov.ph.
5.) After approval of application, get resolution number at Application Division.
6.) Secure Oath Form and pay for metered documentary stamps.
7.) Accomplish Oath Form and verify Resolution No. at Application Division.
8.) Register with the Registration Division or at any of the PRC Regional Offices.
DOCUMENTS REQUIRED:
CHEMICAL TECHNICIAN 1.) Original and photocopies of Birth Certificate (NSO in security paper)
2.) Original and photocopies of Marriage Contract in NSO Security Paper (for married female)
3.) Original and photocopies of Transcript of Records with Special Order ( B ), for graduates in private schools.
4.) Original and photocopies of the College diploma/High School diploma
5.) Original and photocopies of Service Record (government employees), duly notarized
6.) Original and photocopies of Certification of Employment (private employees) duly notarized
7.) NBI clearance
8.) Copy of training to the Board of Chemistry (Sec. 3 of Board Resolution 1989-03)
9.) Certificate of Training
